Estimate GFR with the CKD-EPI 2021 equation

Enter serum creatinine in mg/dL or µmol/L, age and sex. Get the eGFR and its KDIGO category.

The calculator uses the CKD-EPI 2021 creatinine equation without a race coefficient. It divides creatinine in µmol/L by 88.4 to give mg/dL.

G1, normal or high

KDIGO GFR categories
G1 Normal or high90 or above
G2 Mildly decreased60 to 89
G3a Mildly to moderately decreased45 to 59
G3b Moderately to severely decreased30 to 44
G4 Severely decreased15 to 29
G5 Kidney failureBelow 15

One result is not a diagnosis of chronic kidney disease. KDIGO staging needs a reduced GFR or kidney damage lasting more than three months.

How it works

Understand the result

  • Use the 2021 equation without a race term

CKD-EPI 2021 estimates GFR from creatinine, age and sex. It replaced the 2009 version, which adjusted the result for Black patients.

  • Place the result in a KDIGO category

    G1 is 90 or above, G2 is 60 to 89, G3a is 45 to 59, G3b is 30 to 44, G4 is 15 to 29 and G5 is below 15 mL/min/1.73 m².

  • Know the limits of creatinine

Very high or low muscle mass, acute kidney injury, pregnancy and some drugs make a creatinine estimate less reliable. The equation is for adults only.

  • Use the result to support your judgment

A calculator gives a number from the inputs you type. It knows nothing else about the patient, and it does not replace clinical judgment or local guidance.

In a workup

Keep results in date order

Upload the laboratory reports to the patient's workspace in Workup Desk. Ask the record for the creatinine results over time, with the report each value came from.

Questions

Frequently asked questions

Why does my laboratory report a different eGFR?

The laboratory may use an older equation, round differently or cap results above 90. Check which equation the report names.

Does one low result mean chronic kidney disease?

No. KDIGO defines chronic kidney disease by a reduced GFR or kidney damage lasting more than three months.
