Review of systems

Also called Systems review, Systemic enquiry, ROS

A review of systems is a structured set of questions about each body system, asked to find symptoms the patient did not mention.

Why it matters in a workup

The review of systems catches problems outside the presenting complaint. Weight loss or night sweats found here can change the whole assessment.

Focus it on the systems the complaint touches, and record what you asked as well as what you found.

Put it into practice

  • Ask about the systems relevant to the complaint
  • Record positives and the relevant negatives
  • Move a significant positive into the history

Frequently asked questions

Do I need a full review of systems every time?

Not always. A focused review of the relevant systems is usual in a targeted visit. A wide differential calls for a fuller one.
